== Composition == Diatomaceous earth consists of the fossilized remains of diatoms that accumulated over millions of years. It is usually composed of 80% to 90% silica, 2% to 4% alumina minerals, and 0.5% to 2% iron oxide, although the precise composition of every deposit is different. Deposits may contain different amounts of silica depending on the sedimentation conditions, the presence of other sediments (clay, sand, volcanic ashes), and the age of the deposit (diagenesis, silica (SiO2) dissolution/precipitation, diatoms tests ageing). The species of diatom may also differ among deposits. The species of diatom is dependent upon the age and paleoecology of the deposit. In turn, the shape of a diatom is determined by its species. Many deposits throughout British Columbia, such as Red Lake Earth, are from the Miocene epoch and contain a species of diatom known as Melosira granulata. These diatoms have a small globular shape. A deposit containing diatoms from this epoch can provide certain benefits over others. === Burial depth === It is a common misconception that graves must be dug to a depth of six feet (1.8 metres). This is reflected in the common euphemism for death of six feet under. In fact, graves are rarely dug to this depth except when it is intended to later bury a further coffin or coffins on top of the first one. In such cases, more than six feet may be dug, to provide the required depth of soil above the top coffin. In the United States, there is no nationwide regulation of burial depth. Each local authority is free to determine its own rules. Requirements for depth can vary according to soil type and by method of burial. California, for instance, requires only 19 inches of soil above the top of the coffin, but more commonly 30 to 36 inches are required in other places. In some areas, such as central Appalachia, graves were indeed once dug to a depth of six feet to prevent the body being disturbed by burrowing animals. However, this was unnecessary once metal caskets and concrete vaults started to be used. In the United Kingdom, soil is required to be to a depth of three feet above the highest point of the coffin, unless the burial authority consider the soil to be suitable for a depth of only two feet. The earliest known reference to a requirement for a six-foot burial occurred in 1665 during the Great Plague of London.

=== Inflammation === Epithelial cells in Sjögren's disease lesions are active participants in the induction and perpetuation of the inflammatory process. Environmental and hormonal factors, in concert with an appropriate genetic background, are believed to trigger Sjögren's disease, which dysregulates epithelial cells and allows aberrant homing and activation of dendritic cells (DCs), T cells, and B cells. Dendritic cells are antigen-presenting cells that process antigen material and present it to other T cells. Following the migration of lymphocytes into the glands in response to chemokines and specific adhesion molecules, T cells interact with epithelial cells. Epithelial cells are further activated by proinflammatory cytokines (IL-1β, IFN-γ, and TNF), which are produced by adjacent T cells. The early accumulation of plasmacytoid dendritic cells in the target tissues, which produce high levels of type 1 IFNs, seems important, as these cells can further dysregulate the immune response through abnormal retention of lymphocytes in the tissues, and their subsequent activation. IFN-α stimulates the production of B-cell activating factor (BAFF) by epithelial cells, DCs, and T cells. BAFF stimulates aberrant B-cell maturation, leading to the emergence of self-reactive B cells, which locally produce autoantibodies, in a germinal centre-like structure (GC-like), which is also the location of lymphomagenesis (origin of lymphoma).

== Medical uses == NPH insulin is cloudy and has an onset of 1–3 hours. Its peak is 6–8 hours and its duration is up to 24 hours. It has an intermediate duration of action, meaning longer than that of regular and rapid-acting insulin, and shorter than long acting insulins (ultralente, glargine or detemir). A recent Cochrane systematic review compared the effects of NPH insulin to other insulin analogues (insulin detemir, insulin glargine, insulin degludec) in both children and adults with Type 1 diabetes. Insulin detemir appeared provide a lower risk of severe hyperglycemia compared to NPH insulin, however this finding was inconsistent across included studies. In the same review no other clinically significant differences were found between different insulin analogues in either adults nor children.
